HIFU After Ultherapy, Filler, Botox or Threads: The Right Order and Waiting Times

In Ho Chi Minh City almost nobody does just one thing. Clients asking Hugo Beauty about HIFU usually had Botox in the forehead last year, have filler in the nasolabial folds, tried threads once, or had Ultherapy at another clinic and now want to maintain it at a different price point. The real question is not "can I have HIFU" but "which goes first, how long apart, and which areas do we leave alone".

This is the scheduling guide for each of those situations. It does not repeat the general contraindications — pregnancy, active skin conditions, very lean faces — which are in who should not get HIFU. And it does not weigh HIFU against threads or filler; that comparison, and the general principle of combining them, is in our post on HIFU vs thread lift vs fillers. Here we assume you have already had one of them.

The general rule: energy first, injections second

Focused ultrasound creates heat at 1.5–4.5 mm. Those are exactly the layers where filler sits, where Botox acts and where threads are placed. So there are two risks in the wrong order:

  • Heat passing through fresh filler: hyaluronic acid gel can soften, shift or break down faster — you lose part of what you just paid for, and the practitioner cannot tell laxity from replaced volume.

  • Heat passing through freshly injected Botox or newly placed threads: the tissue is still mildly swollen, the product has not settled, the threads have not anchored — it is both more uncomfortable and impossible to judge what each treatment did.

So when you can plan from scratch, the cleanest sequence is HIFU first → wait 2–3 months for tightening to develop → then inject or thread only what is still missing. Many people find they need less filler than expected afterwards, or none.

Most clients, though, do not arrive "from scratch". The rest of this article is for them.

After filler: how long to wait, and which areas to skip entirely

Minimum gap: 4 weeks from the injection date for small volumes (under 1 ml per area). For larger volumes or deep placement (chin, cheekbones, temples), 6–8 weeks is safer — the gel needs time to hydrate and settle.

After that, HIFU is possible, but the technique changes:

  • The injected area is treated shallower or skipped. With filler in the nasolabial folds, HIFU concentrates on the jawline, outer cheek and under-chin, and the 4.5 mm cartridge is not run across the fold.

  • Lips and the lip border: always skipped if there is filler, however old. This is not a standard HIFU lifting area anyway, so nothing is lost.

  • Filler older than 6 months has settled and is very unlikely to move; you still need to tell us where it is so the depth can be chosen.

What to bring: the area, the type of filler (HA or non-HA), the volume and the date. Non-HA fillers (calcium- or PLLA-based, for example) are a different conversation — Hugo Beauty will ask you to check with the injecting doctor before HIFU on the same area.

After Botox: the 2-week gap and why

Botox is not "melted" by heat the way filler is, but there are two reasons to wait:

  1. Botox takes about 10–14 days to reach full effect. HIFU inside that window means you cannot tell whether the smoothness on your forehead is the product or the ultrasound — and if anything is not as expected later, there is no way to know which caused it.

  2. Handpiece pressure and gel massage in the first 48 hours could, in theory, spread the product to neighbouring muscles. A small risk, but no reason to take it.

So: Botox → wait 2 weeks → HIFU, or HIFU → Botox any time afterwards (no wait needed; HIFU does not affect Botox that comes later). If you have Botox every 4–6 months, the simplest plan is to book HIFU just before your next Botox appointment.

The usual Botox areas (forehead, crow's feet, between the brows) are also thin-skinned areas where HIFU uses 1.5–2.0 mm depths, not 4.5 mm. The two overlap less than people assume.

After threads: does HIFU damage them, and when is it safe

Dissolvable threads (PDO, PLLA, PCL) break down over 6–18 months; the real "lift" comes from the collagen scaffold that forms around them in the first 2–3 months. HIFU passing over threads does not cut them, but heat can make them dissolve sooner and inflame the tissue around them — exactly when you need it to stay quiet.

The timeline:

  • Under 3 months after threads: no HIFU on the threaded area. Other areas (neck, forehead) are fine.

  • 3–6 months: possible, avoiding the thread lines. You need to know which way the threads run — ask the thread clinic for a diagram or photos. Hugo Beauty runs HIFU lines parallel to and away from the threads, never across them.

  • Over 6 months, or once the threads have fully dissolved: normal treatment. This is often when clients come to HIFU, as the thread effect starts to fade.

After Ultherapy elsewhere: switching to maintenance with Ultraformer MPT

This is a situation Hugo Beauty sees more and more: a client had Ultherapy one or two years ago, was happy, and wants the next maintenance session at a different price. Because both are focused ultrasound working on the same tissue layers, switching is reasonable — with three conditions:

  1. At least 6 months since the last Ultherapy session. New collagen needs time to form fully; treating earlier puts heat into tissue that is still remodelling and adds nothing.

  2. Bring the details of the previous session: date, areas, number of lines, depths (usually 1.5 / 3.0 / 4.5 mm) if the clinic recorded them. These decide how many shots you need this time and at which layers — usually fewer than a "from scratch" session.

  3. Expect the sensation to differ. Ultraformer MPT fires shots with different energy and point spacing; many clients find it more comfortable, some simply find it different. Our article on whether HIFU hurts describes it area by area.

Three sample schedules

1. Wedding in 3 months. Week 1: HIFU full face, or jawline + under-chin. Week 8–9: review — tightening is now visible; decide whether any filler is still needed in the nasolabial folds (usually less than expected). Week 10: Botox if wanted, filler if needed. Week 12: wedding, everything settled, no swelling. This is "energy first, injections second" in its most practical form.

2. Annual maintenance, already on regular Botox. Pick a fixed point — say March — and have HIFU just before the spring Botox. Repeat next year. The sensible HIFU frequency is in how often should you get HIFU; Botox keeps its own schedule, HIFU simply always goes 0–14 days ahead of it.

3. Two weeks in Vietnam, wanting to do everything. This is the schedule that needs trimming. If nothing was injected recently: HIFU in the first week, Botox in the second, and no filler on this trip (you would fly out before knowing whether filler is even needed). If you had filler just before travelling: HIFU is not for this trip — do it next visit, 4–8 weeks clear of the filler, rather than forcing the dates.

Frequently asked questions

Can I have HIFU and filler on the same day?

In theory yes, because filler injected after HIFU is not exposed to heat. But Hugo Beauty does not recommend it: tissue that has just taken heat is mildly swollen, the injector cannot judge volume accurately, and you will never know whether HIFU alone would have been enough. Waiting 2–3 months and filling only what is still missing almost always looks more natural and costs less.

Will HIFU dissolve my old filler?

HA filler that has settled for over 6 months is very unlikely to be affected when HIFU is run at the right depth and not directly across the injected area. The risk lies with fresh filler (under 4 weeks) and with the 4.5 mm cartridge crossing a filled zone — both avoided by telling us where it is before the session.

I only have one week — HIFU first or Botox first?

HIFU first. Botox after HIFU needs no waiting; Botox before HIFU means a 2-week gap you do not have. HIFU on day one, Botox from day two onwards once the redness has gone.
